If you are renewing in person or by mail, the document must be submitted with your renewal application. Depending on the requirement, when renewing online, you must upload the document before paying for your renewal. If you are exempt from this requirement, you must get a CE-200 form. You must present proof of coverage before a permit can be renewed. If you have a Health Department permit, you must have workers compensation and disability insurance. Workers Compensation and Disability Insurance Payment in person is by debit or credit card, or by check or money order payable to the NYC Department of Health and Mental Hygiene. Customers are encouraged to submit new applications and renewals online. Apply for or Renew a Permit In PersonÄue to the COVID-19 public health emergency, in-person submissions are by appointment only. Online payment is by credit or debit card only. You can also call 311 and ask for "Health Permit PIN." You only need to link to your account once. If you do not have access to your renewal notice, complete a PIN request to have your PIN emailed to you. If you are renewing online for the first time, and you did not apply for your original permit online, you will need the personal identification number (PIN) found on your renewal notice to link your permit record to your online account. Note: Mobile food vending licenses and permits can only be renewed in person or by mail. Follow the instructions for renewing your permit in the renewal notice you will receive in the mail or by email 90 days before your permit expires. You can renew most NYC Health permits online.
